Ooops, the game seems to have crashed.
This achievement is obtained by crashing your Goat Simulator client.
You can do this by pressing the ~ key to open the console, and typing "BeginBVT" (without quotations), and then Enter. The achievement is earned upon reopening the simulator after crashing.
